EOM Self Assessment Tool

This tool is intended to assist cancer care teams assess their readiness for the EOM.  This tool focuses on the four main ares of the EOM. Care delivery, Hierarchical condition categories (HCCs or comorbidities), EOM Measures, and Payment methodology.
